About the Data
Since 2009, reSTART has tracked mental health outcomes using the Outcome Questionnaire-45 (OQ-45) for adults ages 18 and older. This evidence-based tool measures three key domains: emotional distress, interpersonal relationships, and functioning in work, school, or other social roles.
This report summarizes average admission and discharge scores for 236 clients who participated in our intensive treatment program. Six clients were excluded due to missing discharge data.
What We Measure
The Outcome Questionnaire-45 (OQ-45) assesses:
- Symptom Distress (SD): Anxiety, depression, mood instability
- Interpersonal Relations (IR): Relational conflict, social isolation
- Social Role (SR): Functioning in school, work, and daily life
Sample Size: 242 participants (6 with missing discharge data)

Why It Matters
A total score reduction of 19 points exceeds the reliable change index and brings most clients below the clinical cutoff of 63—meaning their symptoms no longer fall in the clinical range. This indicates real, meaningful healing for the majority of clients.

Understanding the Youth Data
Since 2016, reSTART has gathered outcome data using the Youth Outcome Questionnaire (Y-OQ 2.0 SR) for teens ages 13 to 18. This validated tool evaluates emotional well-being, family and peer relationships, and day-to-day functioning—providing a comprehensive view of each adolescent’s progress throughout treatment.
This report summarizes average admission and discharge scores for 70 clients who participated in our residential treatment program. Six clients were excluded due to missing discharge data.
What We Measure
The Youth Outcome Questionnaire (Y-OQ 2.0 SR) is completed by youth and assesses six domains:
Intrapersonal Distress (ID): Emotional symptoms such as anxiety and depression
Somatic Complaints (SOM): Physical symptoms related to emotional stress
Interpersonal Relations (IR): Family and peer conflict
Social Problems (SOC): Rule-breaking, impulsivity, risk-taking
Behavioral Dysfunction (BD): Executive functioning, daily task management
Critical Items (CI): Self-harm, suicidal thoughts, and other urgent concerns

Why It Matters
A reduction in Y-OQ-45 scores reflects real-world functional gains, including better emotion regulation, fewer behavioral issues, and improved safety. At reSTART, adolescents saw an average 38% decrease in overall distress—from a total score of 55 at intake to 34 at discharge.
This change exceeds typical community benchmarks and highlights reSTART’s effectiveness in supporting adolescents struggling with emotional and behavioral dysregulation—often rooted in digital overuse, isolation, or trauma.
